Xanthos rewards are given randomly or do the owners of Xanthos decide?

It's the system that decides, not the owners.

There's theories as to whether each Xanthos has its own mini lottery. For example, if 10 players stroke Xanthos A, then a winner out of 10 gets randomly picked to win a HoP from that Xanthos, and if 8 players stroke Xanthos B, then a winner gets randomly picked from the 8 who stroked that one.
the winner is picked for each Xanthos from all the people that stroked that Xanthos that day. so if 100 people stroked Xanthos A you have a 1/100 chance of getting the HOP. if only 10 people stroke a Xanthos you stroked you have a 1/10 chance. but there's no way of knowing how many people stroked the Xanthos. It's recommended to NOT use the links on people's pages especially really popular players. Everyone are using those links. best thing to do is look up Xanthos in the directory and pick a random one in the middle to the end of the list. make sure it's not owned by popular players. like if you recognize the owners name don't pick that one.
In addition:
* you can stroke 5 Xanthos a day
* you can also stroke your own Xanthos, if you own one
* it's very very VERY rare but in theory, you can win 5x a HoP + 1000 equus on 1 day. Not sure if it ever happened, I've had a few days with 3 (in almost 6 years) but hey, keep hoping :)

There's theories indeed, but it will always be speculation because Howrse never reveals that kind of information.
